Disaster Relief Forms

Form Pick Up & Drop Off

Forms can be picked up at City Hall, the Canada Games Complex, Churchill Pool, NorWest Community Health Centre, VictoriaVille Civic Centre and Oliver-Paipoonge and Conmee Municipal Offices, or downloaded below.

Return forms in person, with your identification, to 401 Donald St. E. (the previous EMS Headquarters), during regular business hours (8:30 am ‐ 4:30 pm).

Clinics Offering Assistance To Complete Forms

Clinics are available Tuesday evening from 6 - 8pm to assist residents in completing their forms. Clinics will be held:

  • Sept. 4, Slovak Legion, 801 Atlantic Ave.
  • Sept. 11, Mosse Hall, 434 Fort William Road
  • Sept. 18, Oliver Paipoonge Township Office Hall
  • Sept. 25, Minnesota Park, 820 Minnesota St.
  • Oct. 2, Slovak Legion, 801 Atlantic Ave.
  • Oct. 9 Moose Hall, 434 Fort William Rd.
  • Oct. 16, Grandview Lodge Auditorium, 200 Lillie St. N.
  • Oct. 23, Minnesota Park, 820 Minnesota St.
  • Oct. 30 Slovak Legion, 801 Atlantic Ave.

Assistance for Losses and Damages Form

Residents who were affected by the flood without insurance or inadequate coverage can apply for assistance for losses and damages under the Ontario Disaster Relief Assistance Program.

Must be completed by Oct. 31, 2012.
