Application Process

    Completed applications must be forwarded to Transit Services by mail or in-person. Incomplete or illegible applications will be returned to the applicant for completion and may delay processing. The completion of this application does not guarantee eligibility. It may take up to 14 days for Lift+ administration to assess an application and determine whether the applicant is eligible for the service. If a decision is not made within 14 days of receipt of the application, the applicant will receive temporary registration until a decision is granted.

    As part of the application process, if it is determined that an applicant may be able to use the accessible conventional transit service, he/she may be asked to schedule an appointment to attend an in-person interview or a transit skills assessment. There is no charge for the assessment.

    Notice of privacy and information practices:

    All personal information, including personal health information, collected is collected under the authority of the Municipal Act, 2001. Personal information is collected in compliance with the Municipal Freedom of Information and Protection of Privacy Act. Personal health information is collected in compliance with the Personal Health Information Protection Act. The personal and personal health information collected is for the purpose of determining eligibility for Lift+ Specialized Transit.
